Client experienced a recent near miss that had the potential to be classified as a dangerous occurrence under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR).

While no formal enforcement action followed, the seriousness of the incident required careful investigation and documentation. The client sought reassurance that their response would withstand scrutiny should a future claim arise. Our Intervention Our Risk Management colleagues provided immediate support by: Assisting with a comprehensive post-incident investigation Producing a robust and defensible accident investigation report Reviewing compliance against statutory requirements Identifying previously unrecognised risk exposures The investigation went beyond basic compliance and demonstrated that the client had taken all reasonably practicable steps to investigate the matter thoroughly.

The Outcome The client gained assurance that they had exceeded their duty in investigating the incident. A detailed report is now in place to assist in defending any future civil claim. Several previously unidentified exposures were highlighted—areas not previously raised by their incumbent consultants. As a direct result of the value demonstrated, the client is now seeking to formally engage our Risk Management team to undertake a full audit with a view to ongoing appointment. This engagement reinforced the importance of competent, commercially aware risk advice delivered at the right time.
